2 Cleanings Per Year (standard membership only, periodontal membership may be necessary for some individual health needs)

2 Examinations per year

1 set of bitewing x-rays

1 emergency visit per year during normal business hours ( includes one PA x-ray)

In office fluoride treatments ( Children only, 2 treatments per year)

20% off all dental treatment completed with 12 months of enrollment if paid in full at time of service (includes any additional x-rays)

This includes elective cosmetic procedures like teeth whitening, crowns, and veneers.

It is only good when used with Eric S. Powell, DDS. Therefore, if you are referred to a specialist, they will NOT offer this discount.

It CANNOT be combined with any other dental insurance or discount.

This is a dental discount plan and is NOT dental insurance.

Dental treatment needed following any type of injury where a lawsuit is filed and therefore outside medical, auto, disability, or workman’s comp type insurances are involved, this discount plan CANNOT be used.

Non-Refundable; no refunds are given if patient chooses not to use their dental membership.

All payments are due at time of service to receive the discount. Any services not paid for at time of service will be billed at the usual, higher fee.

Plan fees are valid only when paid at time of enrollment.

Botox and Juvederm are not included in discount plan.

If paying with cash or check the membership has to be paid in full for the year.

If you choose to extend your payment through Care Credit, the treatment discount is reduced to 10% due to merchant fees.
